  2. Please complete the prospective student-athlete form. Your information will be submitted to the coaching Prospective Athlete Form - Kentucky State University Athletics.
  3. The Kentucky High School Athletic Association (KHSAA) includes both a middle school physical form (you can also go to the KHSAA website and search forms) and a high school physical form (GE04 on the link). Make sure you are using the right one. Parents also must complete the Parent Concussion Signature Sheet. Student Accident Insurance Program.

To individuals filling out the sports application form (below), you must fill out each section correctly and fully, complete with your electronic signature and verification that you have read this application notice and the background check guidelines. The application will then be sent to the Warren County Sheriff’s Department for a background check. If your application is denied by the Sheriff’s Department, the Warren County Parks and Recreation Department will then contact you regarding the rejection.

Any application and background check consent form that is not signed will not be submitted for a background check, and the individual will not be permitted to coach until this is completed.

Goal

It is the goal of the Warren County Sheriff’s Office to check the background of each of the named applicants participating in Warren County Parks Department. The applications must be supplied in paper form by the Warren County Parks Department to ensure the safety of the children and citizens involved in the sports and recreation programs that are conducted in our Warren County parks.

Information Exchange

The Warren County Parks Department shall be responsible for bringing the applications to the Warren County Sheriff’s Office Records Section for the background checks to be completed. They shall be hand delivered, not faxed. It shall be understood that the background checks are second priority to the normal function of the WCSO Records Section. The Warren County Parks Department shall be responsible for making sure that the applications are legible and have the appropriate identifiers on the application for the background check. For a thorough check through AOC records, the applications must have name, any maiden names / previous names, address, at least a partial Social Security Number, and the applicant’s date of birth. If the forms are not legible, the WCSO Records Section will not be able to review or check records on that particular application.

WCSO is only utilizing the AOC’s Court Net program, Kentucky State Police Sex Offender Registry, and local warrant database when checking backgrounds on the Warren County Parks Department applicants. It is understood that WCSO is not able by law to run a full FBI Criminal History query for this process. This system is reserved for Criminal Justice purposes only. Warren County Sheriff’s Office is not responsible for any records or history that may not be discovered during this process. In this agreement, the applicant is giving consent for the WCSO to disclose information resulting from the background check with the Parks Director.

Reasons for Denial

Reason for denial of parks applicants can be but may not limited to any of the following reasons where the applicant has been charged with any of the following offenses. This is not to say that the applicant has been convicted of this offense, merely that they have a charge on their record. Again, it is up to the Parks Director to decide if any applicant can be accepted once the WCSO Records Section has denied the application. Another reason that an applicant could be denied would be for falsifying any information on the application to the Warren County Parks Department or any related paperwork.

The applicant cannot have any of the following on their record:

  • Any active or outstanding warrants of arrest
  • Any felony offense
  • Any offense involving a minor
  • Any offense involving any type of violence or assault
  • Any offense involving the carrying of a concealed deadly weapon (handguns, knives, brass knuckles, and any other item covered by KRS)
  • Any offense involving eluding or attempting to elude police
  • Any offense involving possession, use, intoxication of, selling of, or promoting of illegal drugs or drug-making materials
  • Any offenses involving theft, burglary, or robbery
  • Offenses involving alcohol within the last 24 months (these situations shall be reviewed on a case-by-case basis by the Parks Director)

Accepted / Approved Applications

Applications that have been reviewed and appear to be in compliance with the standards of the background process will be stamped with “WCSD RECORDS” in the bottom right corner.

Denied / Rejected Applications

Applications that have been reviewed and appear to be out of compliance with the standards of the background process will be stamped with “REJECTED WCSD RECORDS” in the bottom right corner.

Appeal Process

Any applicant that was denied by the WCSO during background check may choose to appeal the denial. The denied party shall contact the Warren County Circuit Court Clerk’s Office to obtain information regarding the reason for which the application was denied once the circumstances have been revealed. Once appropriate paperwork is obtained by the denied party, the Warren County Parks Director will reconsider the approval of the application. This process is reviewed and conducted by the Parks Director. Denied parties should correspond with the Warren County Parks Department and not the Warren County Sheriff’s Office. It is understood that once an applicant is denied, the Parks Director requires final case disposition information from Warren Circuit Court Clerk’s Office showing that the charge in question has either been dismissed or expunged. All final participation decision-making authority lies with the Parks Director.

Rules of conduct for all Coaches, Managers, Supervisors, Sponsors, Participants, Players,Administrators, Officials, Parents, and Spectators:


Article I. Coaches, managers, supervisors, sponsors, participants, players, administrators, officials,parents, and spectators shall not:

  1. Curse (malicious or otherwise, verbally, written, or electronically via email, text, social media, orother communication or by physical gesture) at another manager, coach, supervisor sponsor,participant, players, game official, parent, spectator, administrator, or official of the Warren CountyParks and Recreation Department.
  2. Push, hit, strike, or threaten (verbally, written, or electronically via email, text, social media, or othercommunication or by physical gesture) to a manager, coach, supervisor, sponsor, participant, player,official, parent, spectator, administrator, or official of the Warren County Parks and RecreationDepartment.
  3. Be drinking or under the influence of alcohol or illegal drugs, or any controlled substance while onWCPRD or W.C. Public School premises before, during, or after an event.
  4. Act in an irrational manner or make a public spectacle or nuisance of himself/herself before, during,or after a scheduled event.
  5. Violate the rules and regulations of the activity or program in which they are involved.
  6. Participate in any act that undermines league policy or procedures.
  7. Commit in the presence of at least one (1) witness any of the infractions listed in the above-statedArticle I rules (A, B, C, D, E, and/or F).
  8. Commit any infraction not covered under these stated rules but is a violation of league or WCPRDRules and Regulations, Bylaws, or League-amended procedures, guidelines, and/or game rules(example: being ejected from a basketball game due to having two (2) technical fouls).

Article II. Penalties

Violations of Rule A

  1. First Offense: Dismissal from the sponsoring program for the remainder of the season and postseason activities. If violation of the rule in the latter part of the program (twenty percent or lessof the season or post season), the penalty shall carry over to the next season of the sponsoringprogram - No appeal.
  2. Second Offense: Dismissal from Warren County Parks for one (1) year
  3. Third Offense: Permanent dismissal from all Warren County Parks and Recreation Departmentyouth sports-sponsored activities

Violations of Rules B, C, D

  1. First Offense:
  2. Second Offense: Permanent dismissal from Warren County Parks and Recreation Department youth sportsprogram activities. If deemed necessary by Warren County Parks and Recreation Department or League Board of Directors, legal action may be taken against the individual violating the Code ofEthics.

Violations of Rules E, F, G

  1. First Offense:
    1. Players, coaches, managers and umpires: Dismissal for three (3) weeks.
    2. Spectators: Dismissed from all Warren County Parks for one (1) month.
  2. Second Offense: Dismissal from Warren County Parks for one (1) year.
  3. Third Offense: Permanent dismissal from all Warren County Parks youth sports program activities.

Violations of Rule H

  1. First Offense: Dismissed for one (1) game and one (1) week from the dismissal; no contact with team untilsuspension is lifted.
  2. Second Offense: Dismissal from the sponsored program for the remainder of the season.

Anyone in violation of WCPRD Code of Ethics and who has been disciplined accordingly has the right to appealbefore the WCPRD Advisory Board Ethics Committee. Likewise, anyone disciplined by a league for infractions ofsaid league’s rules and/or bylaws has the right to appeal before the WCPRD Advisory Board Ethics Committee.Any and all decisions made by the Ethics Committee will be final and will be enforced by WCPRD.

To participate in JCPS athletics, all of the following forms are required as well as the $20 supplemental insurance premium. This includes the 7-page Physical form, Concussion Statement, Sports Safety Video Form, and a NOTARIZED Parent Permission/Release Form. Crosby Cross Country has a long tradition of excellence. Yearly JCPS Champs, regular Team Top 5 Finishers at the State Meet and many individuals competing in Nationals. We share a close partnership with Eastern High School, training alongside them and eventually moving most of our runners up to State Ranked High Schools teams. More than all that, it's just fun! Our students become a close-knit group of friends that hang on to and love each other for all three years they're at Crosby and beyond.

Field hockey is a fun and fast-growing sport in Jefferson County. Crosby has a successful program of three field hockey teams, including a 6th Grade team, 7th Grade team, and 8th Grade team.
